Who is Eligible?
Age 2-17 Years
The child must be between 2 and 17 years old for at least one day of the calendar year.
Government Benefit
The child, parent, or guardian receives an eligible Medicare payment (such as FTB Part A).

Medicare automatically sends a letter to eligible families. You can also call us to check over the phone with your Medicare card.

Attend Appointment
Bring your Medicare Card. We will conduct the agreed treatments and bulk bill Medicare directly.
